Registration Information (Prices and locations)

To the best of my knowledge, my child is in good health and physically able to participate in an active sports program. In case of an emergency and we are not available for consultation, I hereby give permission for All 'Bout Ball to take whatever measures are necessary for the safety and health of my child and give permission to the physician selected by All 'Bout Ball to hospitalize, secure proper treatment for and to order injections, anaesthetic and surgery. I have disclosed all pertinent medical information regarding prescription medications. I hereby give permission to allow my child's physician to provide the camp with medical information about my child should it be required. I am aware that pertinent information may be shared with appropriate staff on an as needed basis.
